CONTINUOUS GLUCOSE MONITORING (CGM)

If the NHS does not pay for your insulin pump Continuous Glucose Monitoring use and you fund your own, we would like to make this easier on your pocket. We are pleased to offer two CGM Packages available for Medtronic insulin pump users, our Complete and Lite packages, both offering savings on your sensor purchases directly with Medtronic. More information can be found below including the application form which can be returned to our Funding Management team.

The Complete and Lite CGM Packages both represent great value for money and allow you to purchase sensors at a reduced price.

  • The Complete Package is for anyone using sensors all of the time
    If you commit to ordering sensors for a year then you can order your sensors using a discount special code.  Your discount entitles you to purchase boxes of 5 sensors at £210 per box which is a 23.6% discount from the standard price of £275 per box of 5 sensors.  Under this package you also have the opportunity to purchase a replacement Transmitter for the special price of £100 should you need a new one.
     
  • The Lite Package for anyone using them frequently but not necessarily full time.
    If you are not necessarily using sensors continuously with this package you can still order your sensors using a discount special code.  Your discount entitles you to purchase boxes of 5 sensors at £236 per box which is a 14.1% discount from the standard price of £275 per box of 5 sensors.

 

How do I Pay?
Once your account is set up and your application processed, we can take payment by debit and credit card for each order you place.

 

What do I need to do?
Download your application form (link below) and send this through to our Funding Management team at diabetesuk_fundingmgmt@medtronic.com. You will receive confirmation of your package set up together with your special discount codes.

Note - The first time that you place an order you will also need to complete a VAT exemption form. When you submit your application form you will be asked to confirm that have obtained permission and training from your healthcare team to use CGM and will be asked to send a copy of a letter or email of confirmation from your healthcare professional alongside your application form.

MiniMed™ 670G System users must opt for the Complete package due to the nature of SmartGuard™ Auto Mode functions. As the MiniMed™ 670G System Auto mode functions automatically adjust your basal insulin, you will need to discuss your suitability with your Diabetes Healthcare Professional. Your Healthcare Professional will also need to complete a form confirming that they have been trained on the system and will support as well as take responsibility for your use of it.

 

Currently Self Funding CGM with your MiniMed™ 640G and using a Guardian™ 2 Link?
We are currently in the process of transitioning all exsisting self funders using CGM with a MiniMed™ 640G insulin pump and Guardian™ 2 Link to our newest CGM system, Guardian™ Link 3 with Guardian™ Sensor 3. If we haven't been in touch yet, please look out for a communication coming soon.
